Terms & Conditions – (1) You will be contacted to confirm the appointment schedule & payment details. (2) $50.00 credit card deposit is required before the appointment. (3) You may cancel or reschedule any appointment up to 48 hours in advance without penalty. (4) $30.00 reschedule fee will be applied if you reschedule your appointment within 48 hours. (5) $50.00 cancellation fee will be applied if you cancel your appointment within 48 hours. (6) Full payment cancellation/no show fee will be applied if you cancel the appointment within 24 hours or you do not show up on the scheduled appointment.(required)
